Briefing in Washington, DC Examines Results From Alliance to Reduce Disparities in Diabetes

On Thursday, November 20th, The Alliance to Reduce Disparities in Diabetes held a briefing at the National Press Club in Washington, DC.  Briefing participants examined the breadth and results of the Alliance’s work to transform primary care delivery and improve access to quality diabetes care.

  • Creating an innovative link between the clinic and the community to build more supportive environments for patients
  • Developing integrated health system changes that incorporate data-driven, coordinated, team-based care to identify quality improvement strategies that can result in improved patient health
  • Integrating community health workers (CHWs) into the clinical care team to provide better coordinated care
  • Evaluating the payer’s role in helping to create collaborative, flexible and patient-centered health systems that can implement delivery system changes to improve patient outcomes
  • Determining mechanisms for bringing local solutions to scale and discussing adaptability and sustainability in communities across the country
