Call for TEP Nominations: Development of Days at Home Quality Measure

Yale New Haven Health Services Corporation – Center for Outcomes Research and Evaluation (CORE) is currently convening a Technical Expert Panel (TEP).  This TEP will inform the development of a quality measure for patients with complex, chronic conditions spending days at home for implementation in Center for Medicare & Medicaid Innovation (CMMI) models.

CORE is inviting stakeholders (for example, frontline clinicians, patients/caregivers, professional societies, and quality experts) to participate in the TEP.

Given the expertise and mission of your organization, we invite you to nominate individuals who might be interested in participating in our TEP.  The goal is to have broad representation on the TEP to encompass the perspectives of healthcare providers, quality improvement professionals, performance measurement experts, purchasers, consumers, and patients.

Instructions for submitting a nomination are below.  Individuals and organizations should be aware that persons selected for the TEP represent themselves and not their organizations.

CORE is leading this work under contract to the Centers for Medicare & Medicaid Services (CMS) through CMMI.

CORE anticipates holding approximately one to two teleconference meetings between January 2020 and September 2020.  Teleconference meetings will last about two hours.  For more information about this work and TEP member responsibilities, please visit https://www.cms.gov/Medicare/Quality-Initiatives-Patient-Assessment-Instruments/MMS/TEP-Currently-Accepting-Nominations.

CORE is also seeking patients who are aged 18 years or older to join the TEP, but has a preference for patients age 65 and older, since the measure may focus on this age group.  If you know of an interested patient, patient advocate, or caregiver, please forward this information to him or her.

Instructions for Submitting a Nomination

Any individuals interested in participating in the TEP should complete the attached TEP Nomination Form and submit it along with their curriculum vitae and a statement of interest by 5:00pm on January 9, 2019, Eastern Time.  The TEP Nomination Form can be downloaded at the bottom of the webpage listed above.  Please note that in order for the nomination package to be complete, a signature (electronic or handwritten) is required on the TEP Nomination Form.

The documents may be submitted in hard copy to the address below, faxed (203-764-5653), or emailed CMMIDaysAtHomeMeasure@yale.edu.  Once the nomination process is complete, CORE will select a TEP composed of approximately 15-18 individuals based on the areas of expertise and the specific requirements of the measures.
